This course enables individuals who work within health and social care to understand care planning. The course is mapped to unit A/616/6532, T/616/6531 and K/616/6350 of the Level 2 Diploma in Care (RQF).

By the end of this course, the learner will understand:

  • How to prepare to undertake care plan activities 
  • How to support care plan activities
  • How to maintain records of care plan activities 
  • How to contribute to the review of care plan activities
  • The importance of risk-taking in everyday life
  • The importance of positive, person-centred risk assessment
  • The legislation and policies in relation to positive risk-taking
  • How to support individuals to make informed choices about taking risks
  • How to contribute to the support of individuals to manage identifies risks
  • Duty of care in relation to supporting risk-taking
  • The principles of person-centred assessment and care planning
  • How to facilitate person-centred assessment 
  • How to contribute to the planning of care or support
  • How to support the implementation of care plans
  • How to monitor a care plan
  • How to facilitate a review of care plans and their implementation 

Typical duration: 3 hours

*Courses can be delivered as a standalone session, or 2 in 1 day for a reduced day rate. Standalone session rate £350/course. Reduced day rate for 2 courses in 1 day £300/course.

All course prices are for groups of up to 12 delegates and include course material and digital certificates valid for three years.
